Notice of privacy practices.
How the pharmacy uses and discloses health information.
This one is deliberately not drafted. A Notice of Privacy Practices has content mandated by federal regulation and binds the pharmacy to what it says, so it has to come from University Pharmacy's own notice rather than from a template. The page carries the request list instead.

What this notice is
Federal law requires a pharmacy to give patients a Notice of Privacy Practices describing how it may use and disclose protected health information, and what rights a patient has over that information. The required content of that notice is set by regulation.
Why this page is a placeholder
A Notice of Privacy Practices is a legal document specific to the pharmacy that issues it. It has to name the pharmacy's own privacy officer and complaint route, state its own effective date, and describe its own practices — and once published it binds the pharmacy to what it says.
For that reason nothing has been drafted on the pharmacy's behalf here. The notice University Pharmacy already uses in store is the one that belongs on this page.
What we need from the pharmacy
- The current Notice of Privacy Practices as used in the store, in full.
- Its effective date.
- The name or title, phone number and address of the privacy contact.
- Confirmation of how a patient may file a complaint with the pharmacy.
- Whether the notice should be offered as a downloadable PDF as well as a page.
- Whether a Spanish translation is needed.
In the meantime
A copy of the pharmacy's Notice of Privacy Practices is available in store, and by phone on (432) 366-8821.
