The terms that apply to using this website.
Standard website terms for a community pharmacy. Two clauses were left out on purpose because they carry real exposure and are a decision for the pharmacy's counsel, not a designer: a limitation of liability and an indemnity. Ask counsel whether they want them, and in what form.

None of it is medical, pharmaceutical or legal advice, and none of it replaces a conversation with your prescriber or a pharmacist about your own situation. Forms and requests
Submitting a form on this site starts a request; it does not complete it. A refill, transfer or appointment is confirmed by the pharmacy, not by the website. Requests are handled during opening hours (Monday to Friday, 9:00 AM to 6:00 PM; Saturday, Closed; Sunday, Closed). Please do not use these forms for anything urgent.
You agree that the information you enter is accurate and that it is yours to give, or that you are authorised to give it on behalf of the person it concerns.
Prescriptions
Prescriptions are dispensed only against a valid prescription from a licensed prescriber, and subject to the professional judgement of the pharmacist and to applicable law. The pharmacy may decline to fill a prescription where it is required or entitled to do so.
Compounded preparations
Compounded preparations are prepared for an individual patient with a valid prescription and are not FDA-approved. Talk with your prescriber and pharmacist about whether a compounded preparation is appropriate for you.
Products, prices and availability
Product information, prices and availability shown on this site may change and may contain errors. Nothing here is an offer to sell; the pharmacy confirms availability and price at the point of sale.
Your use of the site
- Use it lawfully, and not in a way that interferes with anyone else's use.
- Do not attempt to gain access to any part of it you have not been given access to.
- Do not submit anything false, misleading, or belonging to someone else.
- Do not use automated means to collect information from it.
Content
The text, photographs, layout and design of this site belong to University Pharmacy or are used with permission. You may read, print and share pages for your own personal use. Anything beyond that needs the pharmacy's written permission.
